Overview & Objectives

The Polycare project aims to develop and test and integrated care model supported by advanced ICT systems. These systems and services will allow patients in acute phases to be monitored from home. This methodology strives to achieve the two main objectives:

Enhance quality of life and care for patients.
Increase sustainability of health and social care systems.

Better conditions of life and care for patients as well as sustainability improvement for health and social care systems.

PICASO-POLYCARE co-organized workshop on e-health solutions for home hospitalisation and patient empowerment

During the next 14th of November, POLYCARE and PICASO projects are organizing a workshop on e-health solutions for home hospitalization and patient empowerment. The demand for remote provision of healthcare services has increased in recent years due to many factors, such as the rapid growth of the elderly population in the EU linked together with better quality-of-life standards. Cost-effectiveness of these services therefore becomes a top priority for European governments to improve the sustainability of the European healthcare systems. POLYCARE and PICASO are two running Horizon 2020 research projects, researching and developing different e-health solutions covering different aspects of integrated care. POLYCARE attended to the World Social Work Conference 2018 in Dublin

The World Social Work Conference 2018, organized every four years by the International Federation of Social Workers, the International Association of Schools of Social Work and the International Council of Social Welfare took place from 4 to 7 July in Dublin. On this occasion, the subject matter of the Conference has been “the role of social work in the development of human solutions for new social challenges”. More than 2.000 social professionals, academics, legislators and service users from around the world have attended to discuss key issues that affect their lives and those individuals, families and communities with which they work. Comarca de Somontano has participated representing our consortium presenting our experience in integrated care within the Polycare project. Great profitable time during POLYCARE F2F meeting in Bonn!!

During the past week, the POLYCARE team organized in Bonn the half-yearly Face2Face meeting. The UK Bonn team welcomed the whole POLYCARE team in the universitätklinikum Bonn facilities during two days in which POLYCARE partners discussed on the several topics around the project progress. This time, and facing the last 6 months of the project, discussions were focused on the pilots and the preparation for gathering the indicators for the local and global evaluations. We had also the chance to keep moving with Santélys, which joined the consortium early this year in the preparation of the pilot set up so that they are able to start as soon as possible. In addition, It was also taken the steps towards the finalization of the business, exploitation models and IPR discussion on POLYCARE outcomes. POLYCARE consortium is excited with the work planned for the 6 months of work looking forward to impact integrated care community with the outcomes and results of the project. Stay Tuned! Read

site has been working on a sustainable integrated care business model for 6 years now. There is a strong technological background and well established infrastructure. However in Aragón, home hospitalization is a new concept for which there is growing interest across the entire Aragonese region.
